Sport Specialisation and Overuse Injury: How Physio Helps Young Athletes Stay in the Game
Playing one sport year-round is common in North Shore youth sport, and it comes with a measurable overuse-injury cost. Here is what the research actually shows and how to manage the load.

Playing one sport, on one or more teams, year-round with no real off-season is increasingly the norm in competitive youth sport on the North Shore. It is also, according to the research, a real and measurable driver of overuse injury. This isn't an argument against competitive youth sport. It's a case for managing the total load a growing body is carrying.
Sport specialisation, focusing on one sport year-round, is an independent risk factor for overuse injury in young athletes, with risk rising alongside the degree of specialisation. The American Academy of Pediatrics recommends young athletes train no more hours per week than their age in years (up to a 16-hour general cap), take 2-3 months off from any one sport each year, and limit training or competition to 5-6 days a week. Physiotherapy helps by reviewing a young athlete's total training load across every sport they play, not just the one causing symptoms, and adjusting before pain forces the decision.
What the research actually shows
Studies looking at youth athletes across multiple sports have consistently found that sport specialisation, defined broadly as focusing on a single sport to the exclusion of others, often year-round, raises the risk of overuse injury independent of other factors, and that the risk climbs with higher degrees of specialisation. Serious overuse injuries, the kind that keep a young athlete out of sport for a month or more, follow the same pattern. Female athletes in particular have been found to carry higher overuse injury risk once specialisation and training volume are accounted for.
This doesn't mean specialising is always the wrong choice; competitive pathways in some sports genuinely require focused training at some point. It does mean the injury-risk trade-off is real and worth knowing, rather than assuming more focused training is simply better for performance with no physical cost.
How much is too much
The American Academy of Pediatrics has published specific, practical guidance rather than a vague "everything in moderation" message:
- Weekly hours: overuse injury risk rises once a young athlete trains more than 16 hours a week, or more hours per week than their age in years, whichever is lower. An 11-year-old training 15 hours a week has already crossed the number that matters for them.
- Training load increases: weekly training load shouldn't jump by more than 10-20% at a time. A child practicing an hour a day shouldn't add more than roughly 10-12 minutes a day the following week.
- Days off: children should not be required to train or compete more than 5-6 days a week.
- Time off per year: at least 2-3 months away from any single sport across the year, which doesn't need to be one continuous block; it can be split into shorter breaks.
- Same-day conflicts: if a young athlete plays on more than one team in a season, the AAP recommends avoiding games or practices for both on the same day.
The common thread across all five is the same idea: growing bodies need real recovery time, both within a week and across a year, and the risk of overuse injury climbs measurably once that recovery time disappears.
Early warning signs worth acting on
Overuse injuries build gradually, which means there's usually a window of early warning before they become the kind of injury that forces time off:
- Soreness that doesn't fully settle between practices or games
- Pain that starts partway through a session and lingers for hours or days afterward
- A noticeable drop in performance without an obvious cause
- Reluctance to use or load a specific joint the way they normally would
- Disturbed sleep from pain, or pain first thing in the morning
None of these on their own mean something is seriously wrong. Together, or persisting for more than a week or two, they're worth a proper assessment rather than waiting to see if it settles on its own, especially since a young athlete will often keep playing through early symptoms rather than say something.
How physiotherapy helps before an injury happens
A useful first assessment for an overuse pattern, or for a family that just wants to get ahead of one, looks at the whole picture: every sport and team a young athlete plays, current weekly hours, recent changes in training volume, and any early symptoms already showing up. That total load, not just the sport where symptoms are showing, is usually where the real problem lives. From there, the plan might involve trimming an overlapping season, building strength or mobility in an area that's under-prepared for the current load, or simply making sure genuine rest days and an off-season actually happen.

What's the difference between normal muscle soreness and an overuse injury forming?+
Normal muscle soreness from a hard training session typically peaks within a day or two and fully resolves with rest before the next session. An overuse pattern forming looks different: soreness that carries over into the next practice, pain that shows up earlier in a session each time, or discomfort that starts affecting how a young athlete moves.
