Massage Guns: What the Research Says They Do, and What They Do Not

Massage guns do something measurable, and it is smaller and more specific than the marketing suggests. There is also a list of people who should not use one.

Massage guns do something measurable. It is smaller and more specific than the marketing suggests, and the timing of when you use one matters more than the brand.

At Medstar Sport Physio in North Vancouver, massage guns are treated as a short-term mobility tool. A 2023 systematic review of 11 studies with 281 participants found gains of roughly 3 to 5 degrees on flexibility tests, including 5.4 degrees of ankle dorsiflexion, alongside no improvement in strength, balance, agility, or acceleration, and one study measuring a 3.1 percent drop in jump height. The review advises against use within 5 minutes of a strength activity. A 2025 randomised trial found lower soreness at 48 hours with longer percussion sessions compared with static stretching. There is also a real contraindication list, covering clotting disorders, recent fractures, pacemakers, pregnancy, and use over the spine, neck, or joint replacements.

Here is what the evidence supports, what it does not, and who should leave the device alone.

What the review actually measured

The 2023 systematic review in the Journal of Functional Morphology and Kinesiology pooled 11 studies covering 281 participants, with a mean age of 24.3 years and a group that was 77 percent male.

Range of motion is where the positive findings sit. The Thomas test, which measures hip flexor length, improved by 4.5 to 5.4 degrees at p below 0.001. The 90-90 hamstring test improved by 3.4 to 4.8 degrees. Ankle dorsiflexion, meaning how far the shin can travel forward over the foot, improved by 5.4 degrees at p equals 0.002.

Muscle stiffness also responded, and the review reports that reduction in stiffness was effective at 29 Hz for two minutes or more.

Those results are genuine and they are modest. Three to five degrees is enough to notice on a stiff ankle before a squat session. It is not a transformation of anyone's flexibility.

Where the devices did nothing

The list of null findings is longer than the list of benefits, and it is the part usually missing from product descriptions.

Strength and explosive performance showed no improvements across the studies pooled. One study measured a decrease in jump height of 3.1 percent after massage gun use. On the strength of that, the review authors advise against using a massage gun within five minutes before a strength activity.

Balance, agility, and acceleration showed no meaningful improvements.

Lactate clearance showed no statistically significant difference compared with passive recovery, which in these studies means resting without doing anything.

The authors summarise it as follows. Massage guns could improve short-term range of motion, flexibility, and recovery-related outcomes. In strength, balance, acceleration, agility, and explosive activities, the devices either produced no improvement or produced a decrease in performance.

What the soreness trial adds

Delayed onset muscle soreness, the ache that arrives a day or two after hard training, was tested directly in a 2025 randomised controlled trial in Frontiers in Public Health.

Thirty physically active male college students aged 19 to 23 were split into three groups: static stretching as the control, short-duration percussion at 25 minutes per session, and long-duration percussion at 40 minutes per session. The device ran at 53 Hz with a 6 mm amplitude. Soreness was induced with 10 sets of 7 back squats at 60 percent of one repetition maximum.

Pain peaked at 24 hours in all three groups. At 48 hours, the long-duration percussion group had significantly lower pain than the stretching group, at p equals 0.003. Knee range of motion was significantly greater in the long-duration group than the stretching group at both 24 hours, at p equals 0.006, and 48 hours, at p equals 0.012.

Read that result with its limits attached. Thirty participants, all male, one device at one frequency, and one comparison group. It supports the direction rather than settling the question.

Why the numbers are smaller than the claims

The 2023 review graded the risk of bias in its included studies. Ten of the 11 were rated moderate and one was rated high. Sample sizes ranged from 11 to 55 participants.

Only short-term outcomes were measured. Effects beyond 48 hours are unknown, so nothing in this evidence base supports claims about long-term recovery, injury prevention, or tissue change.

The participants were young, mostly male, and physically active. Whether the same effects appear in older adults, in women, or in people with a current injury has not been tested in this pooled group.

None of that makes the devices useless. It sets the size of the claim that the evidence can carry, which is a short-term mobility effect measured in a few degrees.

How to use one sensibly

The review reports parameters that differ by goal, which is the detail most users skip.

For range of motion and flexibility, use two minutes or less per muscle group at a frequency above 40 Hz. For recovery purposes, use two minutes or more at a frequency below 40 Hz. For reducing stiffness, 29 Hz for two minutes or more was effective. Pressure should sit at around 4 to 6 out of 10 on a comfort scale, which is firm without being painful.

Timing matters. Before mobility work or a warm-up, a short high-frequency pass on a stiff area is reasonable. Directly before heavy lifting, sprinting, or jumping is the situation the review advises against.

The review also warns against prolonged aggressive use over 30 minutes on a single area. More is not better here, and the harm from overdoing it is real rather than theoretical.

Who should not use one

This list matters, and it is not a formality. The 2023 review names contraindications including recent scars, wounds, infections, and bruises, recent fractures and chronic bone conditions, deep vein thrombosis or other clotting disorders, diabetes, neuralgias, pregnancy, fibromyalgia, epilepsy, and pacemakers. It also advises avoiding the face, neck, spine, and areas with joint replacements.

Several of those carry serious consequences. Percussion over a clot or in someone with a clotting disorder is a significant risk. Percussion over a recent fracture or a bone weakened by disease is another. Reduced sensation, which is common in diabetes and in nerve conditions, removes the feedback that would normally tell you the pressure is too high.

The 2025 trial adds a further caution. Its authors note a documented case of rhabdomyolysis following percussion massage use. Rhabdomyolysis is a breakdown of muscle tissue that releases contents into the bloodstream and can damage the kidneys. One reported case does not make the devices dangerous in normal use, and it does support keeping pressure moderate and sessions short.

If any item on that list applies to you, ask your physician before using one. Where a hands-on treatment is genuinely indicated, a clinician can choose between options such as dry needling and acupuncture based on your history rather than on what is in your gym bag.

When to get it looked at

Pain that keeps returning to the same spot, stiffness that does not respond to a few days of gentler training, or an area you find yourself treating with a massage gun every single day is worth having assessed. Reaching for a device daily usually points to a load or capacity problem underneath, and the device manages the symptom rather than the cause.

Some situations need medical attention rather than any self-treatment. Calf pain with swelling, warmth, or redness can indicate a blood clot and needs urgent assessment. So does severe pain after a fall, an inability to bear weight, numbness that is dense and constant, or new muscle pain with dark urine after heavy exercise. Do they clear lactate or speed up recovery?+

The 2023 review found no statistically significant difference in lactate clearance between massage gun use and passive recovery, which in those studies means simply resting. The measurable recovery-related benefits sat in range of motion and in reduced muscle stiffness rather than in blood markers. Effects beyond 48 hours have not been studied at all, so nothing in this evidence base supports claims about longer-term recovery.
